대장암에서 MSI와 hMLH1/hMSH2 면역조직화학염색의 상관관계 분석

dc.description.abstract현미부수체의 불안정성 MSI는 대장암의 탐지등 임상에서 여러 가지 이유로 중요하다. 최근 연구에 따르면 hMLH1과 hMSH2 표현의 면역조직화학의 분석이 MSI - H의 대장 종양의 식별을 위한 신속하고 정확한 방법으로 인식되어지고 있다. 본 연구에서는, MSI와 hMLH1, hMSH2의면역조직화학염색의 상관관계를 알아보고자 하였다. 2008년부터 2009년까지 대장암으로 진단받은 후 대장절제술을 시행한 결장직장암 총 120예 중조직의 양이 충분하고 DNA의 보존상태가 양호하여 MSI 검사가 가능한총 83예를 대상으로 현미부수체불안정성 검사와 hMLH1과 hMSH2에 대한 면역조직화학염색을 실시하였다. 결장직장암 총 83예 중 MSI는 11예 (MSI-H 10예, MSI-L 1예)로 빈도는 13.2%였으며 MSS는 72예 (86.8%)였다. hMLH1은 MSS 총 72예 중 71예(98.6%)가 양성, MSI 11예 중 6예 에서 양성으로 hMLH1 발현의 민감도는 98.6%, 특이도 54.5%였다. hMSH2는 MSS 총 72예 중 72예(100%)가 양성, MSI 11예 중 7예가 양성이었다. hMSH2 발현의 민감도는 100%, 특이도 72.7% 이었다. 전체적인 MSI에 대한 민감도는 98.6%, 특이도는 72.7%였다. 따라서 hMLH1 및 hMSH2에 대한 면역조직화학염색이 MSI의 대체검사로서 가치가 있을 것으로 생각한다.-
